Jane Slayden
Jane Slayden is a Clinical Applications Technician at The Oregon Clinic, PC in Portland, Oregon. She received her B.S. in Speech Pathology and
Audiology from Oregon State University. Mrs. Slayden has been working in medical clinics since 1988 including positions as Health Plan
Coordinator, Front Office Supervisor and Centricity EMR Support. For the last 10 years, she has been working directly with Centricity EMR doing
user support, form building and new user training. She joined The Oregon Clinic in 2005.

Michelle Zamora
Michelle Zamora is the Clinic Administrator for Swedish Urology Group, situated on the main campus of Swedish Hospital Medical Center in
Seattle, WA. Michelle has over 20 years of healthcare and leadership experience in the specialty of urology. She served as president of the
Washington State Urology Society Managers for 2 years and continues to be active on the board. She is also a member of the MGMA and WSMGMA. As
the Clinic Administrator, she is responsible for development, maintenance and training in the electronic medical record. She continues to
dedicate her time to building and managing a strong team of patient-focused, experienced healthcare professionals while overseeing the day-to-day
operations of the practice.

Phil Slover
Phil Slover is the IT Director for Melbourne Internal Medicine Associates (MIMA), located in Melbourne, Florida. Prior to moving to Florida
in 2001, Mr. Slover was the CIO at a St. Louis based company. He was introduced to Centricity EMR as a consultant to MIMA in 2002 and then hired
as a permanent member of the MIMA staff in 2005.

Andrew Hamilton, RN, BSN, MS
Mr. Hamilton is Registered Nurse with six years experience in both in-patient nursing care and outpatient community health as well as nursing
administration. As the Nurse Informaticist, Andrew is responsible for the clinical development of the electronic health records system including
electronic forms, flow-sheet views, imbedded clinical protocols, and other clinical content. In addition, Andrew is responsible for developing
methods to achieve clinical practice integration of the EHRS across participating health centers. Prior to working for the Alliance, Andrew was a
Pediatric Critical Care nurse and also served as the Director of Patient Care Services for Howard Brown Health Center for four years. In
addition, Andrew serves as an adjunct faculty at both Loyola University School of Nursing and the University of Illinois at Chicago School of
Nursing. Andrew holds degrees as a BS in Nursing and MS in Nursing Business and Health Systems Administration both from the University Of
Michigan School Of Nursing.
